    Pessaries are donut shaped rubber rings worn in the vagina, like a diaphragm.  The pessary pushes up the bladder neck against the pelvic bone, and holds it in place, so the natural sphincter mechanism cam work.  It is a simple device, but since it requires some manual dexterity for frequent removal for cleaning, and may interfere with sexual activity, it is generally used in older or sicker patients who wish to avoid surgery.
