Stephan L. Werner, M.D., FA.C.S. The course of bladder cancer is dependant on the pathology of the tumor when initially found. Grade, stage, tumor type, chromosomal duplications, (ploidy) and overexpresesion of the p53 gene, may all play a role. Most patients will have a transitional cell carcinoma, TCC, and over 70% of these will be stage Ta or T1, confined to the mucosa or lamina propria. While these tumors frequently recur, 80% will never progress to a more invasive and aggressive type. Higher stage TCC, with muscle invasion or patients with carcinoma in situ have a higher risk of progression and spread.. Long term survival in patients with muscle invasion, stage T2-T3a can expect a 60-80% long term survival. When the tumor has grown beyond the bladder into the fat, stage T3b. the rate drops to 25-40%. When metastases or other organ involvement are present, stage T4, 5 year survival drops below 20%. Adenocarcinoma, squamous cell carcinoma, small cell carcinoma and cancers metastatic from other organs are not included in these figures |
